The United States health insurance market is experiencing the widest cost shock in recent years. According to several media reports, in 2027 there was a significant general increase in the proposed insurance rates for the ACA (Early Pay Act) in the Länder, and the cost of health benefits provided by employers was the largest increase in 20 years. At the same time, the increased COVID-era tax credit expired in early 2026, leading to the withdrawal of about 3 million people from the ACA market and the cancellation of insurance coverage by HHS (United States Department of Health and Human Services) of about 760,000 registered persons. Even the health workers themselves were not spared by the sharp rise in premiums - a September 2026 study by JAMA (American Medical Association magazine) showed that the average premium increased by 78.4 per cent between 2011 and 2024. This series of changes has had a multidimensional and direct impact in the Chinese community.
In terms of state proposals, the increase in premiums was generally significant in 2027. Colorado, which is projected to increase premiums by about 10 per cent, has been described by the state media as "a further dramatic jump " ; Missouri consumers may face a maximum 26 per cent increase; California's cost of insurance will rise for millions of people; and the population of Cambridge, Massachusetts, will feel the pressure to rise again. The Pennsylvania Insurance Department has officially issued the 2027 ACA rate.
The pressure is equally evident at the level of employer insurance. Mercer ' s survey of 1,800 employers shows that the cost of health benefits per employee is expected to increase by 8.2 per cent in 2027, the largest increase since 2003, and the fifth consecutive year. Two thirds of companies with more than 500 employees plan to increase their share of the premium. According to the United States Bureau of Labor Statistics, employers paid an average of $3.48 per employee per hour in June 2024, or nearly a quarter of their total benefit expenditure of $14.07. According to industry sources, this expenditure directly squeezes the space available for wage growth.
There are several clues to the loss of coverage. HHS cancelled insurance coverage for approximately 760,000 persons registered in healthcare.gov, but DC Health Link in Washington, D.C. was not affected by the federal cancellation. New York State reduced its coverage to about 450,000 free this summer, and the fate of the affected population was the focus of attention. In a fraudulent clean-up operation, the state of Hawaii may lose insurance benefits to hundreds of residents of Obamacare.
The core context of these policy changes is that the enhanced COVID-era premium tax credit is not due in early 2026. Previously, the ACA market covered some 9.2 million people (government data as of February), and subsidies were based on income calculations. After the expiration of the tax credit, premiums and out-of-pocket payments increased significantly, resulting in the withdrawal of about 3 million people from the ACA market. A partial exiter moves to a limited range of short-term health plans or health-sharing schemes - - Oregon state regulators have publicly warned of the risks of such alternatives. The industry giants have also warned Congress that, unless action is taken, premiums will continue to climb.
JAMA published a study in September 2026 that provided key data: between 2011 and 2024, the average health insurance premium in the United States surged by 78.4 per cent, from $4,008 to $7,151 per year, double the overall inflation rate of 39 per cent over the same period. In 2024, 10.5 per cent of medical support workers (e.g. nursing assistants, physiotherapists assistants, dental assistants) were not insured, more than twice as many as 3.8 per cent of medical practitioners and technicians were not insured. The non-insured rate is 11 per cent for all adults aged 19 to 64.
Specific cases further illustrate the seriousness of the problem. Dr. and Mrs. Durham, Idaho - Family Medical Doctor Joshua Durham and Pharmacist Ashley Durham - Prior to the purchase of insurance for themselves and two children through the ACA market, the premium for comparable schemes this year rose to nearly US$ 1,600 per month, and they decided to abandon insurance for the first time, to use $50,000 from the Health Savings Account (HSA) to cover medical expenses, and to attempt to establish a material exchange system with photologists to obtain eye examinations. The monthly premium for another nurse increased from $700 to $1,500, and although family members retained insurance owing to their medical condition, they were forced to abandon dental insurance and give priority to the dental needs of their children. The Starbucks Cafe also indicated that a second job was being considered because of the soaring insurance costs.
For the American people, the impact of these increases is broad and concrete. The Chinese who buy insurance through the ACA market will be faced directly with the 2027 state-to-state rate increase and should carefully compare the specific terms of the different schemes and the out-of-pocket ceiling during the open registration period. Those who rely on employers to provide insurance are concerned about whether employers are increasing their contribution share and assessing their own maximum monthly expenditure.
Chinese who leave or are unemployed are exposed to the risk of an insurance break — for example, workers in St. Albans Creamery, Vermont, whose insurance is due to expire next week — and trade unions have requested court intervention. In similar cases, the option of renewal and special registration period provided by the state or former employer should be known as soon as possible. The warnings of the Oregon Regional Regulatory Authority are noteworthy: some cheap alternatives (such as short-term schemes and health-sharing schemes) have limited coverage and should not be considered complete alternatives.
Small business owners can focus on the new tax credit policy for small businesses providing health insurance (as reported in Connecticut). For households with HSA accounts, the programme of using savings to cover medical expenses when premiums are excessive can be assessed, subject to the rules governing the use of HSA funds and the tax implications. Overall, in an environment of rising premiums, early planning, careful comparison of the terms of the plans, and understanding whether they are eligible for special registration periods or subsidies are key steps in safeguarding American health care.
